We are seeking a compassionate and organised individual to join our Hospice Rapid Access Service (HRAS) as a Care Coordinator. This role is vital in ensuring that our patients receive the highest quality of care by coordinating and managing their healthcare plans.

Location: Basildon Hospice

Salary: £27,000 - £30,100 depending on experience
Hours of work: Full-Time 37.5 hours per week
Contract: Permanent

Key Responsibilities for Care Coordinator

The primary duties of the Care Coordinator include:

• Developing and managing comprehensive care plans for patients.

• Coordinating with healthcare professionals to ensure continuous and consistent patient care.

• Monitoring patient progress and adjusting care plans as necessary.

• Providing support and guidance to patients and their families regarding healthcare needs and services.

• Maintaining accurate and up-to-date patient records.

• Organising and scheduling appointments, treatments, and follow-ups.

• Facilitating communication between patients, families, and healthcare teams.

• Ensuring compliance with all healthcare regulations and standards.

Qualifications and Experience Required

We are looking for candidates who demonstrate the following:

• Previous experience in a healthcare or care coordination role.

• Strong organisational and multitasking abilities.

• Excellent communication and interpersonal skills.

• Proficiency in Microsoft Office applications (Word, Excel, Outlook).

• Ability to work both independently and as part of a team.

• Knowledge of healthcare regulations and best practices (desirable).
