St. Luke's Hospice is a locally-based charity providing compassionate care for individuals living with life limiting conditions. We are currently seeking a highly motivated Community Nurse Team Leader to join our team in Hospice Community Services. The ideal candidate will lead a team of Community Nurses that will be providing palliative and end of life care in the community, with passion and dedication, ensuring the services run smoothly.
The role is known internally as Locality Team Leader.
About the role
The Community Nurse Team Leader will manage a team of Community Nurses ensuring the provision of high standards of care to patients and their families/carers who have a palliative care need.
Salary: £23,572 to £29,925 (£39,287 to £49,875 per year full time equivalent)
Contract: Permanent
Hours: Part-time, 22.5 hours per week, Monday to Friday
Location: Basildon Hospice and working in the community in South West Essex
Key Responsibilities:
- Providing leadership and guidance to the team in Hospice Community Services.
- The post holder will support the teams to ensure the delivery of the right care at the right time to individuals with a palliative care need in the community.
- The post holder will have a commitment to collaborative working, actively supporting and liaising with other health care professionals and agencies.
- The post holder will have responsibilities ensuring high quality care is delivered across Hospice Community Services, supporting the patient journey to ensure appropriate and seamless care.
- To facilitate effective communication within the team and across departments to promote collaborative efforts.
- To assist in audit and research projects, particularly within the service, taking a lead on specified projects as appropriate.
- To monitor and evaluate team performance, providing guidance and support where necessary.
- To foster a culture of continuous improvement and professional development within the team.
Skills and Experience:
- You will be a Registered Nurse with demonstrable post qualification experience, including palliative care or related specialities.
- Have the ability to motivate and unite a team towards common goals.
- You will have exceptional organisational and problem-solving skills.
- Excellent communication and interpersonal abilities, as well as a sound knowledge base linked with holistic assessment and advance care planning.
- You will have a passion for making a difference in the community.
- Have experience in assessment skills and non-medical prescribing or have willingness to undertake these.
- Experience in supervising junior staff and pre-registration students.
